Chart Instructions

  1. Edit or Write: You can click on the highlighted fields in the PDF to type your child’s name, or print it blank and fill it out together by hand.
  2. Printing: Use the print icon on the top right of the viewer, or download the file and print from Adobe Reader.
  3. Printer Settings: Make sure your printer is set to “Fit to Page” or “Scale to Fit” for the best results.
